If one receives better pay if less is done for the patient (capitation) there is one type of adverse motivation. If one is payed better for invasive procedures than "cognitive" services, or if one is payed better by having the patient seen more often (current fee for service) this is a different type of adverse motivator. (the second is probably more insidious because our patients often sense that more care is better care, and this is certainly not always the case) Perhaps I shouldn't even excuse the salaried provider as the motivation there can be to monkey with the job description to reduce work load and responsibility while maximizing salary! The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
