Re: 20 weeks with PPROM + previaFrom: Ronald Helm (ronhelm@ronhelm.seanet.com)Mon Jul 1 15:47:28 1996
A 16 year-old primigravida at 20 weeks (menstrual history and 8 week ultrasound) arrives with documented preterm premature rupture of membranes (PPROM), not in labor, and a non-bleeding complete placenta previa on ultrasound. The diagnosis of placenta previa at 20 weeks and asymptomatic is quite common and the parapatetic placenta "wanders away" from the cervical os as the lower uterine segment develops. Induce this patient by whatever means you normally would and anticipate that she may have more than the average blood loss associated with second trimester abortion. By any means AVOID a hysterotomy which would be comparable to a Classical C-section and commit this woman to repeat C-sections for future child-bearing. ronhelm@ronhelm.seanet.com

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
