Could someone forward the original message concerning these twins to me? I seem to have missed it. I have a particular interest in discrepant growth. We had a similar case three years ago that resulted in one twin weighing 72% of the larger at spontaneous delivery at 35 weeks. The discrepant growth was first noticed sonographically at six weeks (LNMP = 5.9 weeks, twin A = 7.1 & B = 6.6 weeks by CRL). This discrepancy continued until after the 24th week, when it began increasing slowly. By the exam in the 34th week there was a 4 week discrepancy. This case is described in detail in Chapter 7, Size/Age Analysis of FETAL SONOGRAPHY, W. B. Saunders, 1996. Both twins were considered normal, just different sizes. They were still different sizes at 2 years age. Thanks, Terry J. Why such a discrepancy ?????

The second baby with Biophysic profile 5/8 with desacelerations type 2 and the first baby with biophysic profile 4/8 with desacelerations type 2

I have forgotten Type 1 & 2, but I think type 2 were late(Hyoxic) decels when we used that terminology. If so the length of time that they were occurring (24 hrs?) would have caused significant problems and probably lead to the poor outcome. You didn't tell us what the babies died of ???? Intraventricular hemorrhage, hyaline membrane disease, severe anemia from abruption ????

In our unit an 800gm baby with a decent apgar has a 65-75% survival and a 1200 gm baby even better than that. With those odds one would be pushed to deliver them if the onset of fetal distress was in the recent (10-30 min) past but not if it had been going on for 24 hours.
