I would like to know your opion about a case at my duty last weekend.

A 23 years old G2P0. Gestational age 24 weeks with premature labour. Dinamic 3/10 minutes strong, did not stop with Ritodrine (we dont use MgSO4). The cervix dilated til 7 cm. USound, 1st baby vertix/transv. Fetal weight #1- 800 g/#2- 1200g The second baby with Biophysic profile 5/8 with desacelerations type 2 and the first baby with biophysic profile 4/8 with desacelerations type 2

It is a twin pregnancy diaminiotic/dichorionic.

My collegue gave corticoid and after 1 day performed BP profile and monitoring which is described above.

He decided to perform a c-section.

The outcome was bad. Baby #1 died at 5 minutes after delivery and baby #2 the day after.

What are your opinion in this case, what would you do?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
