>I am currently in transit and was wondering if anyone on the list could tell Shelley: I believe ACOGs opinion is that it is up to the primary surgeon. We have recently struggled with third-party payers refusing to pay for an assistant, even in difficult cases. Plus, we have had an HMO refuse to pay for a perineorrhapy (in conjunction with a vag hyst, anterior/posterior repair, etc) because they consider it "plastic and cosmetic surgery!" I recently did a Lash core vag hyst (500 gram uterus), ant/post repair, Raz, and sacrospinous suspension, and was told "sorry, we're not paying the assist fee because assistants are not necessary for vaginal hysterectomies." This was a tough case, and we wrote a terse letter asking them to provide documentation that ob/gyns are routinely doing such cases without assistants. They could not, and paid. When will it end?! good luck Ashley D. Ashley Hill, M.D. dahmd@gate.net Orlando, FL

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
