I believe ACOGs opinion is that it is up to the primary surgeon. We have recently struggled with third-party payers refusing to pay for an assistant, even in difficult cases. Plus, we have had an HMO refuse to pay for a perineorrhapy (in conjunction with a vag hyst, anterior/posterior repair, etc) because they consider it "plastic and cosmetic surgery!" I recently did a Lash core vag hyst (500 gram uterus), ant/post repair, Raz, and sacrospinous suspension, and was told "sorry, we're not paying the assist fee because assistants are not necessary for vaginal hysterectomies." This was a tough case, and we wrote a terse letter asking them to provide documentation that ob/gyns are routinely doing such cases without assistants. They could not, and paid.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
