Re: Breastfeeding questions

I would appreciate any help that y'all could offer me with the following breastfeeding questions: 1. Does anybody have any insight into the ability of a patient with "streak ovaries" to breastfeed following delivery. This patient has had total exogenous estrogen support prior to conception and conceived via donor, being supported through the first trimester with exogenous hormonal therapy.

Don't know for sure, but I have had a lady who had Pan Hyst at 2 mo post partum for invasive cervical cancer. Continued to fully breastfeed until baby past a year. My understanding is that estrogen plays a fairly minor role in lactation. If her pituitary and thyroid function is adequate, she should do OK. Sure won't hurt anything to try, but should have a lactation consultant in mind if problems.

2. Does anybody have knowledge of patients with triplets breastfeeding. I know that many pediatricians discourage breastfeeding twins, but I have had several moms do just fine. I have advised the patient that it really depends somewhat on her supply, but if the demand is demonstrated the supply should be there. Any comments?

In the olden days of wet nurses, there are reports of women bf 3-4 infants at a time for several years. My only thought is that she better have a nice easy chair cause that's where she will stay for the majority of her time.

Finally, does anybody have the resource for the support groups for multiple pregnancies. I know that they are out there, I just need their addresses.

LaLeche League would be the resource with the longest record of experience in this area. They have a web page, I think. Do a yahoo search. You could briefly subscribe to lactnet, post your questions and get a multitude of answers, then get off (HUGE and extremely active list). If you want to subscribe, let me know email.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
