We have seen two technical problems when using misoprostol for ripening/induction. First, the pill is so small it frequently adheres to the finger when the digit is removed from the vagina (especially if the pill is broken into pieces to help dissolving). Second, when it is inserted with KY there appears to be a barrier effect that can prevent the pill's dissolving. If KY is used we typically find a soft but intact pill still unabsorbed in the vagina up to two hrs later that can actually be massaged into the vaginal wall. Interestingly if a gel is made (20 gtts saline + 1 pack of lubricating gel + miso <50 or 100>, mixed in a medicine cup on the floor and given like PGE2 gel) the pat seems to respond nicely. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
