As you probably know, Kaiser is a huge HMO with many hospitals in different cities and states. Each hospital operates fairly independently. At my hospital in Redwood City (a Level II hospital in a suburb of San Francisco) vaginal breech deliveries are almost never done. Only the rare aftercoming twin or late-discovered breech is delivered vaginally. At the San Francisco Kaiser (a Level III institution with a residency program -- home of the illustrious perinatologist Terry Jones, familiar to all on this list) selected vaginal breech deliveries are done.

Generally speaking, in this area private doctors tend not to do vaginal breeches. At UCSF and San Francisco General Hospital (which is affiliated with UCSF and the site where I trained) vaginal breech deliveries in selected cases are done. In my last practice, which was a private practice with deliveries at two private Level III hospitals in San Francisco, our consulting physician did do vaginal breeches with the CNM in attendance assisting. There is a widespread fear of the perceived legal risk associated with vaginal breech birth. I have heard many physicians say that they would like to do them, but don't because of the risk of malpractice litigation. Such a shame that our ability to offer choices to birthing women is limited by these sorts of factors.
