Gary: The administration of therapeutic dosis of I- 131 to a pregnant women, can damage or ablate fetal thyroid gland, this occurs specially after 11-12 w when fetal thyroid gland begins to function and concentrate iodine. <I have heard of cases of administration of thyroxine to fetuses who have <been exposed to I-131 after the 1st trimester: <1. Is fetal thyroxine replacement necessary before delivery? <2. Can the dosage and efficacy of the fetal thyroxine replacemnt be accurately assessed? < 3. Is the replacement therapy effective? To my knowledge T3 and T4 don't pass placenta. TRH administration to the mother is the way to stimulate fetal thyroid tissue. In this case, the best option seems to do surgery, and wait until delivery for I- 131 Gustavo Izbizky Servicio de Obstetricia Hospital Italiano de Buenos Aires Buenos Aires, Argentina ghizbiz@italob.fmed.uba.ar

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
