Matenral thyroid cancer

Gary Kleinman inquired regarding the use of radioactive iodine after thyroid surgery for papillary carcinoma of the thyroid. My vote is surgery and deferral of the radioactive iodine until after delivery. This is a slow growing tumor. The studies on fetal thyroid replacement are limited to case reports. Although normal values for TFT's are available on fetal blood sampling, the frequency of sampling is unknown. I would not destroy the fetal thyroid gland. The long-term neurologic outcome of hypothyroidism in utero is unknown.
