"re 25 weeks, transverse lie, prom, arm presentation, FHR = 80 I would start with general anesthesia, then if she is almost fully dilated - internal podalic version, total breech extraction else, cesarean delivery."

I would opt for a stat CS. Fear to great a potential for trauma associated with internal podalic version and breech extraction in a 25 weeker.

If you do not have a high tech. ICN across the hall from L&D as I do, these heroics may be futile.

Agree with David Nagey that mortality is 40 to 60%. Am also concerned with long term morbidity which by my experience and observation exceeds 60%. Does anyone have 5 year follow-up on these kids (fuctional as well as medical)?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
