Joe, Dave et al,

I agree essentially what has been said, however, but I think I would at least get a few more data points to convince me that her fasting is truly eleveted such as three capillary AM fastings with a diet history to be analyzed for CHO content. Treating one value could be "dangerous." All said, if her values remain elevated, then I would treat. If they are not, then I would "spot" check her once a week at most for another fasting capillary glc. If it is elevated again, start the Rx. Nobody has addressed the issue of fetal surveillance though. I would probably treat this situation as a GDM (whatever your protocols are wherever you are!). Don't blast me too much for this one... I'll be out of town for the next few days looking for a job. Cheers

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
