I had a patient with a similar complaint after an enblock pelvic resection for endometriosis. She had previous surgery for scoliosis with a spinal fixation. Her symptoms were cyclic and occurred during cycle days 10-14 each month. Dr. Benson in Indianopolis, a gyn neurologist measured pelvic, vaginal and perineal evoked responses. Specific abnormalities were present. He had a similar case occur after an auto accident with pelvic fracture.

My patient was somewhat relieved by Lupron for a brief period of time. Then her symptoms recurred and an MRI, showed compression of the cauda equina and cord compression higher up. Since repeated back surgeries, she reports that her symptoms are now more bearable.

You might consider a trial of Lupron or even cafergot to minimize the sensation of pelvic engorgement. I am intriuged by the possibility of a frontal lobe cause. Please keep us informed of this patients progress.
