agree with plan..

would nerve studies as with perineal studies in incontinence evaluations shed some light on the "source" of the trigger? I do not know the answer but do you think the same testing equipment might find a reflex loop or central stimulus or prove it is generated locally? if so.. would that info help your choice of treatment?

You mentioned the two things that came to mind to me already.
