Clem Brown, my oncologist colleague who works out of Cambridge, and I would be grateful for any help and advice you can give us. The patient is a 33 year old para 1 with a 12 year old daughter. Ten months age she presented with a tumour which on histology proved to be a clear cell carcinoma of the endocervical canal stage one. I carried out a Wertheims Hysterectomy. Very soon she developed a pelvic recurrence which was treated by a full course of radiotherapy. Next there was an isolated deposit 3mm across in the anterior wall of the vagina which I excised, and this was quickly followed by another pelvic recurrence which is half was to the pelvic side wall on the left and has already reached the side wall on the right and is fixed to it. It has progressed anteriorly so that it is now invading the bladder and can be clearly seen on cystoscopy. Can anyone suggest a useful treatment that has a reasonable chance of helping her and that does not increase her misery without conferring worthwhile benefit, please?
