Laryngeal Carcinoma and Pregnant

I would like to present this one for discussion:

34 y/o G3P3A0LC2 at 25 weeks gestation presenting for continued prenatal care at our high risk clinic...

On 3/1/96 she underwent total laryngectomy, bilateral radical neck dissection, and partial thyroidectomy for a squamous cell carcinoma of the larynx.

Final staging T4N3 (stage 4 disease)

She is now left with a permanent tracheostomy and a feeding tube.

Prior medical history is significant for cardiac surgery at 7 for a "hole in the heart". No current cardiac symptoms...

OB history significant for SVD, C/S for breech, and a VBAC (third child died of "crib death" at 3 months)

Radiation oncologist wants to proceed directly with XRT, but patient is unaware of her survival with or without radiation. Additionally, she is unaware if XRT is delayed until a more mature fetus is prematurely delivered (ie 32 weeks) how that might adversely affect her survival..

We are planning radiation onc and perinatal cnference to discuss these issues and offer recommendations..

Any thoughts out there?
