Check with the Ca. Med. Assn. They have a booklet and consent form already in use. We have adapted this for MO. and are in process of publishing, soon. Your state Med Assn. can do this for you (collectively speaking) and make it a statewide form, thereby increasing the acceptance. They can also modify it to conform to your state's law.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
