ACOG has a small book, pale blue entitled Preconception Guide. It has a 2 page inventory the patient and her husband fill out detailing genetic, family and personal history. It includes diet, drug, alcohol and smoking information and has information about the cycle, mucus, ovulation, and fertile times. It's very good. Acog distribution center can provide it for you.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
