vulvar vestibulitis

I have a patient with persistant tenderness and erythema of the left Bartholin's gland orifice easily reproducible by touching this area with a q-tip. She has dysparunia only referable to this spot and there is no tenderness or mass of the Bartholin's gland itself. Exam is suggestive of vulvar vestibulitis. She was initially treated with pre-coital 2% lidocaine jelly but has developed decreased effect now. She has no evidence of subclinical HPV and, except for point tenderness, has good sexual function. She is at the point now of desiring excision of the vestibule. Does anyone have experience with the long term results of this procedure? I have read success rates of anywhere from 50% to 85%. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
