Re: 36 weeks and creat increasingFrom: MNudel@aol.comSun Mar 31 08:12:58 1996
El and all, Uric acid 8.2 2 weeks ago, now 10.2. 24 hour urine results with 4000cc over 24 hours showed 75cc/min creat clearance. Repeat labs just prior to induction, SGOT 39 platelets 167K, creat 1.2, uric acid 9.2. Given the creat clearance of 75, despite the fact that this woman looked great on exam with BP's in the 80's diastolic, after discussing with my perinatologist we opted to recommend delivery. Four hour labor with healthy 6 lb 5 oz baby, no RDS. Will obviously follow her post partum for recovery of her urinary function. Any predictions on how long it will take her urinary insufficiency to resolve? I agree I wouldn't have checked a creat either but that's what you get when your a consultant. I also agree it doesn't make any sense and that underlying renal disease must be a consideration. She did have a creat of 0.8 two weeks ago however. Went ahead with delivery as my perinatologist and every other ob in my area thought I was crazy to sit on her at nearly 37 weeks. Just don't like sitting out on a gangplank in a storm by myself. Gotta admit I feel better with her delivered. Thanks for the input. Mitch
