It sounds like this lady needs a visit from Bard Parker, via a vertical midline incision, and an attempted bilateral ovarian cytectomy, with frozen section if necessary. I would not find a CA-125 level at all useful in this patient, because a level of any amount (high or low) would not change my management, and because I don't believe CA-125 levels are currently useful in premenopausal patients.
