We have the same problem here in Baltimore. The HMO would rather send the patient to the capitated radiologist. The radiology group got so overwhelmed that the HMO altered its policy and let us do emergency sonograms if the patient is bleeding or if ectopic is suspected.

Make sure that the patient is fully aware that the inconvenience of going elsewhere for her ultrasound is the result of the HMO's policy and that you have no control over the situation. Most patients still feel that we as physicians have some control and it's not until the patients themselves start protesting, will some of the changes of managed care be reversed.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
