Re: Anovulatory infertilityFrom: Greg le toubib (ovadoc@enter.net)Wed Mar 13 13:34:04 1996
Dear Dan, I'm a reproductive endocrinologist so I use Clomid like most OB/GYNs use birth control pills. You can give clomid for 5,7,10 days at 50.100.150 mg a day. There are studied of prolonged us up to 30 days with 100mg and certainly literature that suggest in a large woman (ariynd 200 lbs) 200mg of clomid may work. The problem is that you can get them to ovulate at those doses but the pregnancy rates are low because of the antiestrogen effects of the drug. Classically in the US we start clomid on day 5-9 to give a 30 day cycle. Practially as long as you know you"started"a cycle you could start the drug. That is really day 3 of bleeding. I get an ultrasound exam done one week after the last pill. A good follicule should be 20-25mm in diameter. Normal is 18-25mm bur clomid makes bigger folliciles (Pergonal could have a mature follicule at as little as 14 mm). If you have a good follicule in size and a good esrtadiol in the mid to high follicular range for your lab I would give 5,000 units of HCG. 10,000units can down regulate. A seum progersterone done a week later is okay. If you have quesation do a biopsy 2-3 days before expected memses. Hope this helped Greg Lang
