Dr. Kahaled Homouda asked if a patient undergoing spontaneous abortion at 6 weeks EGA is at risk for Rh sensitization. I would agree with Dave Nagey that the chances are unlikely but Rhesus immune globulin is probably indicated. I would not however agree that the patient is out of the woods until a titer is determined in her next pregnancy. She may belong to that group of "hyperresponders" that become sensitized with as little as 0.1 cc of RBC's. Bowman calls these individuals "sensibilized". In these cases a titer may not detect low grade sensitization but B lymphocyte memory cells are present. A titer would then become positive in a subsequent gestation when the patient is once again exposed to the antigen. Ken Moise Maternal-Fetal Medicine Baylor College of Medicine

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
