Hear, Hear, I also risk pontification on this subject but must agree wholeheartedly with you. The failure to train residents in the art of operative VAGINAL delivery is a serious problem that is leading to poor care. I too have seen Board Certified obstetricians deliver a transverse arrest at 2+ station abdominally simply because they are not trained or are not willing to take the perceived medicolegal risk. I don,t know what the solution is on a national level, but at least at this institution we actively support rotational forceps delivery, midforceps delivery and breech delivery with the correct prerequisites and indications.
