Slightly off the subject, but still linked to the point about 95% epidurals: what amazes me whenever visiting US centres is that nobody seems to have heard of Nitrous oxide ('Entonox'), which is usually the first choice of analgesia during labour here. It is self-administered, hence 'empowering'; it is also uplifting -in fact some of our ladies say they are as high as a kite.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
