Dear Dr.Lisse:

I have worked with midwives for most of my 15 year practice, plus residency. Their care has been excellent . Midwives offer an alternative that appeals to a great many women. The health care that is delivered by these providers is outstanding. However they consult or turn to us (M.D.s) for instrumented delivery and C-section at the same or slightly lower rate than our own patients.

When you comment that U.S. obstetrical care is not affected by our hugley divergent population, and that your country is as diverse as ours; I can only conclude that you have not really travelled in the Unithed States. We have inter-city areas of poverty that are the size of some European capitols. In some of our cities the number of hard core drug users exceeds the population of other countries.

When we solve our problems of poverty, drug abuse and racism then we will have perinatal and neonatal statistics that equal or exceed (pardon the American spirit here) Japan and some of the more enviable european countries.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
