>>Some frustations you've alluded to may be particularly acute for midwives,
because many patients have chosen us assuming we will endorse anything they want. In these cases risk/benefit counselling falls on deaf ears.

Although all such refusals are carefully documented, this has not been shown to make a difference when it comes to litigation.>>

I agree, and am likewise disturbed when a woman chooses one path (one of the choices given, might not be my first choice), and then is unhappy later that things didn't go the way she wanted...or why wasn't this or that offered, even thought they were not appropriate...or why didn't you try to sell me your viewpoint !! The situation I am thinking of, the mom had a lot of anger which she never shared with me, despite some opportunities at postpartum exams, and with my partner. How can I explain myself if never given the opportunity?? Like "we didn't offer forcepts after two plus hours of pushing, because the baby's head was too high...and it would have been dangerous..." Or, that I wasn't with her, even though I was there the whole time, helping her and encouraging her. I try to give my clients the opportunity too discuss things about their experince, to clear things up. Unfortunatly, right after the birth is usually too soon, and time slips away after that. How do others of you deal with the aftermath of a particularly complicated scenario?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
