I have a great case for the list members.. 41 y/o G4P4 female presented with low abdominal pain and severe nausea and vomiting.. KUB noted distended loops of bowel with air-fluid levels c/w obstruction. U/S of the pelvis noted a 6cm left adnexal mass that was complex and cystic.. Free fluid was noted. The patient had peritoneal signs and was getting no relief with NG suction. Patient taken to laparoscopy and was found to have a segment of ileum herniated through the left broad ligament. This was reduced. General surgery looked at the bowel and did not want to do a bowel resection.. The defect in the broad ligament was corrected by LSO with an endoscopic stapling device. This AM the patient is a new woman... Her bowel function has returned and she is ready for D/C home. The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
