I would make (a) optional. I think it probably depends on a lot of factors, including institutional experience. To put it first and absolute is a little strong (but it SHOULD be there, obviously).

For (b), how about an incomplete breech? If the butt is there, even if there's a foot next to it....????

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
