I saw a patient in the ER today, 33yo G1P0 at 16 weeks with 24hr hx worsening (now severe) lower abd pain. Started like menstrual cramping radiating to anterior thigh. Exam and Ultrasound show posterior (tender) mass (myoma) 6x6cm with irregular echos. Slight fluid in cul-de-sac. CBC shows slight elevation of WBC. All other labs WNL. Assuming this is degenerating intramural leiomyoma but curious about the expected course of condition and whether anyone has seen anything other than a 7-10 day course of gradual improvement. This woman is a nurse heading for a remote part of Alaska in a few weeks for a public health service rotation. That's heroism.
