I'm an advocate for health care reform because in my 35 years I've watched too many patients having to make choices that were unreasonable due to the system as it is. Until that reform occurs consider this concept.... fee for service. Every time I mention this people can't seem to hear the words - fee for service aka no insurance company! I have an internal medicine bud who has never filed insurance. He started practice about 1980. His small staff has an automated system to give the paper work to their patients to submit for reimbursement. He has the smoothest running office I've seen. He is allowed more time with his patients and truly knows them. I don't live in a tiny town (Kansas City) and he doesn't take produce for payment but it works. When I was opening my independent sonographer's office I was complaining about blue cross and blue shield not giving a provider number to nonphysicians (other than physical therapy). He said why bother. You can keep your fees lower and minimize your headache by simply being fee for service. I am so glad I listened. More people should consider taking the leap. You won't get referrals from the insurance company, but the stream from you friends and colleagues will be the same.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
