It's all about hubris and money.

The academic gurus make their pronouncements, and they are never wrong. I might make their pronouncements a bit suspect if they're ivory tower gurus and don't actually get their hands dirty. [I don't make this a global statement -- I hold great respect for such as Seb Faro, who could make a guru pronouncement as the blood dripped off his "in the trenches dirty hands."]

Then, there's the money. (1) Community hospitals have to decide what the bottom line is for providing "immediate availability" (e.g. residens, a Laborist, whatever); and (2) malpractice lawyers generally tend to turn down cases in which the time frames/protocols/pathways are clearly met.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
