I wish to emphasize the role of cigarette smoking on placental abruption and fetal death. (1) Did she smoke while on stress?

I wish a happy new year to all listmates.

A nurse friend's  daughter was in the hospital for 8 days,  2 weeks after losing her job (that apparently was due to pregnancy discrimination over several months) . She was admitted for contracting at about 29 weeks, cervical change to -2/80%/-3 cephalic, TVU showed decreased cervical length to 1.1cm, put on mag sulfate for 3 days and then observed till she was sent home  still contracting mildly about 11 times an hour.    Fetal monitoring showed the fetus was ok the whole 8 days. I find it unusual that she was discharged with continued  contracting lasting 80-120 seconds. Her first 2 BPP showed AFIs of 12.27 and 14.2 but was discharged with an AFI down to 8.3. BPP also showed hyper and hypoechogenicity of the placenta-is that from the infarcts?   Mom came back about 11 hrs after discharge with terrible pain and fully dilated. Delivering doc diagnosed abruption because copious amounts of blood came out with delivery of the head. No placental culture was done but path report showed 30% of the placenta was infarcted! Baby ok with Apgars 9/9 so apparently no damage was done. Baby is 7 weeks old now.   I know stress can cause preterm labor but not abruptions, right? Mom is 35 years old with her first pregnancy-very healthy with no physical issues.   Gail

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
