it took me an hour and forty five minutes to do a tvh last week. big uterus, had to morcellate to reach the upper pedicles.the bladder was glued to the anterior surface of the uterus. the posterior peritoneum was way high, took forever to get to the cul-de-sac. then there was a little pumper on the right pedicle row, don't know how it slipped out. i resutured the pumper, did a pursestring for the peritoneum, then closed the cuff with figure of eight sutures. i was wringing wet with sweat, and thanking god for stable vitals from the recovery room, and a good hemoglobin the next morning. i have done a 20 minute tvh, but it seems like the ones i remember are like this one. verner nellsch, md
