I would have expected a larger drop in Hgb if there was a large hemotoma, if you are getting watery dischage there could be also a componet of vault/ vaginal cellultitus, I almost always start on Flagyl 500 mg TID X10 days. You could open it up but you may not get much with such a small Hgb drop. Vault cellutlitus will respond very quickly to antibiotics. If you open it up under general anesthesia probe it with your finger gently and then use a large pack in the vagina to pack externally not in the dead space itself.
