First I would make sure she understands that she is putting her life at risk with a singlton pregnancy let alone a twin pregnancy. If she insists on getting pregnant I would tell her her best chances of a good outcome would be with a singleton pregnancy. It sounds like her children are all healthy, you would think after the monoamniotic twin IUFD she would get some insight that sometimes your luck runs out.
