I'm hoping to open my prenatal clinic in about 2 weeks (finally!) and am having a hard time finding patient registration forms and health history forms that new patients fill out in Spanish. If you use these in your practice, could you please share them with me? Ideally, I'd like to have 2-3 so I can make up my own. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
