The reason for suggesting the transverse upper segment incision is that
it heals more strongly so ultimately is less likely to rupture in a
future pregnancy. I put the incision as close to the lower segment as
is possible, but still allowing enough room for the fetal head to get
through.

Saludos Steve - I feel much, much more comfortable with a vertical
incision in those cases - never a regret (Gracias a Dios) exactly at
what level do you make the incision?
