type of incision?

Several times in the eighties and nineties I had the pleasure and privilege of attending a general OBGYN CME course sponsored by the St. Barnabas Medical Center, located in New Jersey, and held in NYC, with Dr. James Breen, department director, serving as course moderator...always a worthwhile course, with fine presentations and speakers.   One of Dr. Breen's always popular slides showed a nice view, up to the liver and stomach, visualized through a transverse incision extended to the iliac crests...his throwaway line (He always had a lot of them, he's a great speaker and presenter!) was that at the end of the primary case they let the intern do an appendectomy..and the intern complained about exposure!   Over the years I have remembered the value of a transverse incision, in morbidly obese patients, properly extended...first shown to me and other residents by a gyn onc attending...info available through Google scholar, no doubt, out of the Green and Gray journals. I don't think it pays to be rigid and dogmatic over what has to be done in what circumstances...lots of ways to skin a cat.   Just an opinion and..well, y'all know the rest.   H

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
