Re: vbac on demandFrom: Garry E. Siegel, M.D. (garrys@mindspring.com)Wed May 14 12:38:31 2008
As Lynn said, we all must accept some type of responsibility for the unattached/ER/walk in/drop in patients. Charles eloquently pointed out some of the issues with them. They are not inherently bad patients or people, or necessarily lawsuit happy, of course, but they are people with whom we have no prior relationship, and they may be as unhappy with their circumstances as we sometimes are. I would guess that they wish they had doctors, weren't out of town, didn't have insurance, or, in my area, were likely illegal immigrants who can't/won't access the system. Who knows? I think that most of us would accept a role as being physicians for these patients if there could somehow be some adjustment to the model, i.e. a common sense approach that says "The doctors are going to take care of these people fairly and equally, and the hospitals and government are going to be reasonable in how it all works." Thus, for the example cited, Verner needs some type of reasonable compensation for his time and effort and perhaps some type of indemnification. Even if those exist, the aggravation and trouble he must face are but two of the reasons why people are dropping Ob. We could go on for days on this. . . Garry

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
