What is your policy on VBACs? What is the policy of the hospital regarding VBACs? The anestheisiologist? I would get as many people involved, have her, her SO, mother, father, or whomever else is with her, sign the ACOG statement that recommends against VBACs in that exact situation. Calling the local Justice of the Peace or Judge is a good idea too. I would also tell her that she is not getting any augmentation. Lastly I would try to find another OB who is comfortable with VBACs (as I personally don't do them ever) and if cannot be found document that as well. You might also call your insurance company and let them be aware of the situation.

question--you are an ob/gyn on call for a small community hospital (3 delivery beds) with the or crew and anesthetist at home on call. a 26 y/o g3p2 comes to the hospital, with a history of 2 previous cesareans, the first for cephalopelvic disproportion. she is unknown to you or the other ob/gyn practicing at the hospital. she is 1 cm, ruptured, at term. she demands a vbac, and refuses to have a repeat cesarean.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
