left upper quad entry with a 5 mm direct visualization scope. i find it easier to go under adhesions rather than through them. agree with the attempt at vaginal hyst; at the other extreme, the robot is great for lysis of adhesions near bowel and other vital structures. bowel prep no matter what approach. would rather do a hand assist procedure than abdominal hyst.
