But if she is dense as it sounds, once you are able to get your hand in all
the adhesions are down. Unfortunately not every one has access to the
daVinci, but you are right about the adhesions. The only drawback with the
robot is no articulating harmonic scalpel

left upper quad entry with a 5 mm direct visualization scope. i find it
easier to go under adhesions rather than through them. agree with the
attempt at vaginal hyst; at the other extreme, the robot is great for lysis
of adhesions near bowel and other vital structures. bowel prep no matter
what approach. would rather do a hand assist procedure than abdominal hyst.
