Depends on exam, sometimes I make the decision with examination under anesthisa and tell the patient that she will only know after the surgery. Have done lots of vag hysts with greater than 2 c/s, some of the most difficult have had 1 c/s or none, can't predict what pathology will cause most problem sometimes severe endometriosis and no problem, sometimes multiple Leeps make for difficult vag Hyst, basically I tell patients its vag Hyst possible abdominal hyst.
