At 38 6/7 with srom one hour and favorable cervix it is still reasonable to await onset of labor. The practical approach however would be to start pitocin induction. (or possibly cytotec 50mcg )vaginal). GBS should be available by this time. IF not antibiotic prophylaxis according to risk factors. IE prom 18 hrs or preterm <37 weeks. she meets none. Therefore no antibiotics at thsi point. > Date: Thu, 18 Oct 2007 06:41:37 -0500> From: eramirezt@coqui.net> To: ob-gyn-l@dns.obgyn.net> Subject: Term PROM> > 34 y/o, GI, 38 6/7 weeks - SROM at 8PM - arrives at LDR 9PM - 1-2> cm/75%/-1- soft. almost no contractions...baby fine... last visit 35> weeks - GBS done - results not available.. options..> > 1) therapeutic rest for the patient and Doc - no antibiotics> > b) same as 1) but your wife tells you to start her on ampicillin - you> agree.> and wait till 8AM - start her on pitocin for induction/augmentation> depending on cervical status - you either decide not to start/start her> on antibiotics -- nummm - it's Sunday.> > 2) Go to LDR - bring with you your latest book, start Pitocin,> antibiotics and your reading.> b) same as 2) but no antibiotics - fell asleep - no reading.> > 3) same as all of the above - different induction agent...> > Ef> > --> “ The greatest obstacle to knowledge is not ignorance, > it is the illusion of knowledge.” Daniel J. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
