If her cervix is 3 cm dilated from an aborting myoma, I suppose there must be plenty of prostinglandins floating around which could not be good for her heart. If her condition negates surgery at this time, temporization is all you can do. I would consider giving this patient Lupron (although it may take a long time to work) and transfuse her to keep up with the blood loss. If this patient has to die, the OR table would be the worst place. I would also suggest checking her cervix for bleeders frequently since she had a LEEP not long ago and the cervical dilatation could be tearing more vessels.
