John-They give more inclusion criteria: to be included on NQF’s list of “never events”, an event had to have been characterized as: Unambiguous—clearly identifiable and measurable, and thus feasible to include in a reporting system; Usually preventable—recognizing that some events are not always avoidable, given the complexity of health care; Serious—resulting in death or loss of a body part, disability, or more than transient loss of a body function; and Any of the following: Adverse and/or, Indicative of a problem in a health care facility’s safety systems and/or, Important for public credibility or public accountability.

The case I posted yesterday was that same neuro's 2nd wrong side surgery- the units third in 6 yrs... so how long should we keep reimbursig that kind of care?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
