Re: OB- DVT prophylaxisFrom: Garry E. Siegel, M.D. (garrys@mindspring.com)Sat Jul 7 06:27:23 2007
FWIW, at Northside in Atlanta (17K private deliveries/year), this issue has been discussed in committee in light of the push (JCAHO?)for prophylaxis throughout the facility. A very good Nurse Clinician/research person pulled articles and there was no consensus found as to how to address this issue. Obviously, a low-risk, routine laboring patient is at risk for DVT, just as a elective/scheduled C/Section is, and that doesn't add in other factors such as obesity or thrombophilia. That said, available interventions--LMW Heparin (Lovenox) and Sequential Compression Devices haven't been tested in this population (if my memory serves), and, perhaps importantly, I think that many pregnant patients would be reluctant to get the Lovenox when laboring, and SCD hose just don't seem too practical for laboring patients, to say nothing of comfort. This post makes me start thinking about using SCD hose on sections, however--relatively easy to use and put on OTW to the OR for the scheduled or unscheduled sections. What do others think? Garry

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
