No sense reinventing the wheel. I have a monograph from the APOGO educational series, it was released in 2001. It gives a great algorithm, that they adapted from Levichek et al, 2000, it's also referenced in Motherisk: http://www.motherisk.org/prof/updatesDetail.jsp?content_id=348 The monograph also has a good reference table on nutritional support of the hyperemesis patient, adapted from Hamaoui and Hamaoui, Gastroenterology Clin N Am., 1998; 27:89-121. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
